Symptoms for blown cylinder head gasket?

What are the indicatioons that the cylinder head gasket is blown-
more or less (Scorpio Diesel 95 2.5 155000 km)

loss of coolant/oil/cylinder pressure
One or more of the following
possibly overheating/low oil pressure
possibly loss of power/performance
possibly burning oil from exhaust
possibly steam from exhaust
possibly oil and water mixing
possibly coolant leaking externally from the gasket
possibly very high pressure in the coolant system
