Can anyone help with FINIS code

I am at work so can't check myself - wonder if anyone can look up the clutch fitted to my 95 2l Ultima?
I am after the finis, and description - booked it in for the morning and the garage said some thing about a concentric type so I want to make sure he has the right one before they start the job!

Dave, I've checked with my local FMD and there is a problem without the index no so if you want to PM me I'll try again. He has given me one for a 2ltr dohc with output of 115 and that is a Finis No of 5029608. If it helps if I ring you then PM me the details. Regards, Mike.
