Intermittent rough idle 24v

Hey all, For about a year I have been experiencing occasional rough idle. Idle runs between 400-800 rpm and jerks the car when stopped at a light. When it is really bad I have to put the transmission in neutral. The car has about 116000 km on the odometer. The problem only occurs once in a while (a couple of times one day and then nothing for a week) which leads me to think that it is not a problem with vacuum hoses or such. I have checked all the rubber hoses and they seem fine. Also, the car doesn't seem to lack power. It can happen with the engine cold or warm. However, I did get a diagnostic reading that said the engine was running lean. Could it be a bad sensor or sticky solenoid problem?? But then wouldn't the diagnostics tell me of any faulty sensor? Thanks.

** UPDATE ** I cleaned the MAF and also checked the IACV. The idle still runs rough sometimes. So no change. When I remove the IACV when the engine is idling badly the problem seems to go away. So I will try to get a new IACV and see if this really is the problem.

Have we checked vacuum hoses yet? Even a weak joint or small crack can be difficult to spot on cursory inspection and there's a 'T' junction (or two) that is a known problem on the 24V. (?) (From what I've read here anyway ) I only mention it as there was another topic recently about rough running as it was vacuum hoses IIRC.
